How to Obtain an AP Handbook and Application

Obtaining the most current Advanced Practice Handbook and application is easy. All you need to do is download them by clicking on the link below. Note that all forms must be typewritten and mailed to the WOCNCB office.

Using Interactive Forms

We encourage candidates recertifying via Advanced Practice to submit your application portfolio electronically. You can access interactive Microsoft Word forms for Wound, Ostomy, or Continence below.

Use one set of AP Forms for each specialty, save the files on your computer and send the electronic files as e-mail attachments to info@wocncb.org.

The interactive forms are organized by sections for Wound, Ostomy and Continence and include point logs and summarization/verification forms. Extra verification forms are included for the most commonly used point categories.

• Just skip pages you don't need
• Fill in the gray, shaded areas. They are unlimited and will expand as you type.
• Use Tab key or Arrow keys to move the cursor into each shaded area.
• Type "x" in the checkboxes you are filling in.
• If mailing, print only the pages you fill in.
