CWOCN's CAUTI Prevention Plan Brings Evidence-based Practice to the Forefront

Since the October 2008 announcement about payment restrictions by the Centers for Medicare and Medicaid Services, I've been thinking about prevention of pressure ulcers and catheter acquired urinary tract infections (CAUTI). I've also taken a few steps further than just thinking about it.

I work at a 110-bed acute care facility in Tennessee. Working together with Jenny Evangelist, RN, BSN, APN, CWOCN, and a team made up of the infection control coordinator, education director, and a clinical nurse specialist, I co-authored a three-step, evidence-based approach to decrease CAUTI at my facility.

We began with an investigation and review of the current practices in our facility related to the insertion and maintenance of catheters and staff knowledge of CAUTI. This information was necessary so we could understand what areas we needed to focus our efforts on in order to decrease or eliminate CAUTI. We also performed a review of the current literature and a meta-analysis of relevant research. This took several months and resulted in the development of a three-step, comprehensive approach to the prevention of CAUTI.

We piloted the program on one of our medical surgical units and in the emergency department in mid-May. Part of the pilot included education of staff nurses. We expect that we'll need to make a few changes before it can be introduced to the rest of the facility. Because most of our plan was adapted from existing plans found through our literature research, we believe it can be adapted to fit any facility.

I did not include the plan outline in this article because I will submit our research to the Journal of Wound, Ostomy and Continence Nursing. However, I wanted to share this experience with other Board Certified Wound, Ostomy and Continence Nurses. Like me and my research team, nurses want to make a difference in patient outcomes. Using the knowledge, skill, and experience I have gained in as a WOC nurse and nearly five years as a CWOCN, I have been able to take positive actions that can help us all solve the CAUTI problems often prevalent in our acute care and long-term care facilities.

The Value of Board Certification: One More Tool in Her Pocket

She's an AP nurse and Family Nurse Practitioner. She owns her own successful business. She's an acknowledged expert in continence care and co-chaired the development of the Scope and Standards of Practice for Advanced Practice Continence Nurses. What more could she want?

To be a WOCNCB Certified Continence Care Nurse.

To understand why, we'll need to go back a few years.

Laura Croft Maw received her Family Nurse Practitioner credentials from Brigham Young University in Provo, Utah in 1996. After 10 years in family practice she got the urge to be her own boss. Meeting up with some advanced practice continence nurses at a conference was a turning point. "I was really intrigued," she remembers. "I'd been doing a lot of continence work at my clinic. We had five male physicians and they loved sending me their female patients who had continence problems."

Laura trained with some continence nurses and soon began her own practice on a part-time basis while still working full time at the clinic. Soon the demand for her skills in continence care was so great that she quit her job, and she's been providing continence care full time since 2004. "Aside from the long-term care facilities, I have my own outpatient clinic where I get referrals from physicians, home health care and assisted living facilities," she adds.

Business is growing and Laura attributes that to a new trend. She explains, "There seems to be a paradigm shift in continence care from only ten years ago. People had the notion that as you got older, it was accepted and expected that you'd develop continence issues. However, this is not the case. Even the CMS guidelines F-315 tag specifically states that 'incontinence is not a normal part of aging'. With the new F-315 guidelines, people are becoming more aware of the problem and realizing that much can be done to improve the condition."

Laura is seeing long-term care facilities respond to this F-tag and its guidelines. They are ramping up care to prevent the negative sequelae — UTIs, skin breakdown, and falls — related to bladder health. Long-term care facilities are highly regulated and, surprisingly have recently taken the lead as the most regulated entity in the nation.

Another trend Laura is seeing relates to the Baby Boom in general and the desire of Boomers to have a better quality of life. "It's dignity and cost," Laura explains. "Many families are seeking out continence care for parents and loved ones as well as for themselves."

Which brings us back to the question: Why does Laura Croft Maw want to be a WOCNCB Certified Continence Care Nurse?

"It's another tool in my pocket," declares Laura. "I learned about the Certified Continence Care Nurse credential — the CCCN^® — through networking with other Advanced Practice CCCNs. I believe we should always increase our level of understanding, and as I'm studying for my CCCN, I'm learning more and extending my knowledge base."

When Laura sits for her CCCN exam, she'll be demonstrating her commitment to quality patient care. As she reminds us, "AP continence nursing is an emerging role that's grown in respect. To continue with this work, we have to provide validation of our abilities. Certifying won't necessarily change my scope of practice, but it will provide validation and recognition that there is a body of knowledge I am certified in along with its associated skill base."
